Our spine is made up of numerous small bones. Each of these small bones is individually known as a vertebra.

Between every two vertebrae are pressure-absorbing discs, which separate one vertebra from another, helping with movement. Between the two bones is a soft bone (intervertebral disc), which acts like a car spring or shock absorber.

Any kind of problem in these bones or discs causes pain in different parts of the body, especially in the waist and neck. Inside the spine is the spinal cord. Nerves spread to the organs of the body through this cord.

Causes of pain: Disc displacement usually occurs due to various reasons including lifting heavy objects, trauma, jerking in certain body positions. This can cause pressure on the adjacent spinal cord or nerve root or both. In lumbar disc prolapse, the patient experiences severe pain in the lower back or groin. As a result, the patient has difficulty sitting or standing.

Nerves originating from the spinal cord in the waist extend to the legs. So, in the initial stages, besides back pain, pain can be felt anywhere up to one or both sides of the thigh, knee, ankle, ankle or toe.

Apart from this, tingling, numbness, sensation in the legs decreases, gradually the legs become weak and at some point the patient finds it difficult to walk, stand or even sit. On the other hand, nerves originating in the neck (cervical) extend from the neck to the hands.

So neck pain in the early stages of cervical disc prolapse can be accompanied by pain in the right or left arm or both arms. As with lumbar disc prolapse, there is tingling and numbness in the hands. Hands may become weak, even both hands and feet may become weak.

Treatment: Conventional treatment involves relieving pressure on the nerve or nerve cord through surgery. Apart from this, the pain can be kept under control by taking regular physiotherapy. In addition, percutaneous laser disc decompression (PLDD) can be used to reduce the pressure by applying a certain level and type of laser beam.

This allows the displaced (prolapsed) disc to return to its original position and the patient can recover by reducing the pressure on the spinal cord and nerve roots. Besides, it is possible to fix the damaged nerve through laser optothermo mechanical stimulation.

Laser treatment does not require incisions and sedation of the patient. As a result, there is no such problem in the case of diabetes or heart disease. Most patients with disc prolapse in the developed world no longer undergo surgery.

The type or level of laser beam used in this treatment does not have any side effects. In Bangladesh, laser treatment is now used to treat back pain, including neck and back pain.
